To begin the repatriation from Perth to Italy, the following documents are required:

  • Complete and submit our online forms under Repatriation Documents to allow us to take the deceased into our care.
  • Original passport held by the deceased or Italian Identity Card.
  • Original death certificate with an Apostille and translation into Italian.
  • Coroner’s report with translation in Italian.
  • Appointment of a receiving agent or funeral director in Italy.

Documents required by the Consulate General of Italy in Melbourne must be emailed at least two weeks in advance and presented at your appointment.
